Notice of Availability of IRS Form 1095-C for Tax Year 2025

As an Applicable Large Employer (ALE), The City of Moorpark is required to offer health insurance coverage to its full-time employees and report this information to its employees and to the IRS. This information is provided on IRS Form 1095-C, Employer-Provided Health Insurance Offer and Coverage. Beginning with the 2025 tax year, The City of Moorpark will provide the Form 1095-C to you upon request, at no charge.

How to Request:

You may request a copy of your 2025 Form 1095-C by emailing Noriko Wu at nwu@moorparkca.gov or by calling (805) 517-6242. Once requested, your form will be mailed to you within 30 days.
